At Axelson Chiropractic Health Center, we get you going quickly. Our new patient procedure only takes one visit that is typically 30 minutes or less. We start with necessary paperwork, a consultation with one of our doctors, and either your first adjustment or the scheduling of X-rays (if needed).
Save Your Time and Ours
Cut down on the time spent on your first time in the office by printing off and filling out the new patient paperwork at home. If you are unable to print at home, we will give you our paperwork packet to complete when you arrive on your initial appointment. The packet includes a general welcome sheet, a place for you to put information on why you are coming in, and necessary forms for HIPPA and insurance.
Wear something that allows comfortable movement, preferably pants or shorts. If you plan to use insurance, make sure you bring in an insurance card and valid driver’s license.
